NYC Medicaid fee-for-service enrollees, Family Health Plus enrollees who are 19 or 20 years old, and NYC Medicaid Managed Care enrollees (except those in a Managed Long Term Care plan) in the following Medicaid Managed Care plans:

  • Affinity Health Plan (code 82)
  • Amida care (code OD)
  • Emblem Health (code 99) (formerly the Health Insurance Plan of Greater NY [HIP])
  • Healthfirst PHSP (code SF)
  • Health Plus, an Amerigroup company (code KP)
  • Metro Plus (code 92) (Metropolitan Health Plus) and Metro Plus (code OM)
  • Neighborhood Health Providers (code NP)
  • NYS Catholic Health Plan (code SP) (also known as Fidelis)
  • United Healthcare Community Plan (code MO) (formerly AmeriChoice NY)
  • VNSNY Choice Select (code VS)
  • Wellcare of NY (code WC)
  1. Enrollees in a Managed Long Term Care plan.
  2. Individuals who are not NYC Medicaid fee-for-service enrollees or who are not NYC Medicaid Managed Care enrollees.
  3. Family Health Plus enrollees who are not 19 or 20 years old (19 and 20 year olds can get transportation).
  4. Enrollees who live within 10 blocks of their medical practitioner and who can walk there.
  5. Enrollees who are able to utilize mass transit and are attending a medical practitioner who participates in the Public Transit Automated Reimbursement (PTAR) system, should not request transportation services from LogistiCare.

Ask your medical practitioner to contact LogistiCare to get you a ride by a livery, ambulette, wheelchair, stretcher or non-emergency ambulance.

Yes, but LogistiCare will have to speak to your medical provider. Your medical provider must say that you need to come in today and that your treatment cannot be delayed to another day. Otherwise you will have to reschedule your appointment.

You or your medical practitioners can start calling:

  1. April 9 for enrollees who need a ride to a Brooklyn medical practitioner on or after May 1, 2012.
  2. June 18 for enrollees who need a ride to a Queens medical practitioner on or after July 1, 2012.
  3. August 13 for enrollees who need a ride to a medical practitioner in Manhattan or the Bronx on or after September 1, 2012.
  4. September 17 for enrollees who need a ride to a Staten Island medical practitioner on or after October 1, 2012.

You or your medical practitioner should contact LogistiCare. LogistiCare will contact your medical practitioner and ask them to make all of the trip reservations for you. LogistiCare will make sure that you get rides until you or your medical practitioner says that you don't need them anymore.
If your ride is more than 15 minutes late call the "Where's My Ride" line 1-877-564-5923. LogistiCare will send another vehicle to get you if necessary.

You have the right to appeal denials of rides by LogistiCare. Following the completion of LogistiCare's complaint process, all members have a right to request a State Fair Hearing.
A Fair Hearing is a chance for you to tell an Administrative Law Judge why you think LogistiCare’s decision is wrong.

There are several ways to request a fair hearing. You may submit your request online, by U.S. mail, by telephone or by fax.

  1. Bus/Subway if:
    1. You live within 10 blocks of a bus or subway stop
    2. Your medical facility is within 10 blocks of a bus or subway stop
    3. You can walk 10 blocks
    4. You understand common signs and directions
  2. Ambulatory/Livery if: You can’t take the bus or subway and can walk unassisted from your residence to the curb and board the vehicle
  3. Ambulette if: You can walk but you need the assistance of trained personnel to walk to and board the vehicle
  4. Wheelchair if: You are confined to a wheelchair and require a lift-equipped or roll-up wheelchair van and the assistance of trained personnel
  5. Stretcher if: You cannot walk, are confined to bed, cannot sit up or sit in a wheelchair but you do not require medical attention during transport.
  6. Non-emergency Ambulance if: You cannot walk, are confined to bed, cannot sit up or sit in a wheelchair and you require medical attention and/or monitoring during transport.

  1. May 1, 2012 for enrollees whose appointment is with a Brooklyn medical practitioner.

You need your Medicaid ID number (this is the ID listed on your Medicaid card formatted as 2 letters, 5 numbers, 1 letter; like XX12345X). You will need the name, address, zip code and telephone number of your medical practitioner. You will need the street address and the telephone number where you want to be picked up. You will also need the date and time of your appointment, any special needs you have and the general reason for the appointment (doctor's visit, check-up, eye appointment, etc.).
You do not have to contact LogistiCare but your medical practitioner does in order for your ride to continue.  However, if your driver is late, call Where’s My Ride at 877-564-5823; the call taker will then contact your transportation company, and determine why the driver is late.
